Step-by-step guidance to help you complete our questionnaire. It's the foundation of your estate plan — the more complete your answers, the better we can protect you and your family.
Why We Use a Questionnaire
Our Estate and Asset Protection Questionnaire is the starting point for every estate plan we prepare. It captures the information we need to design a plan that's tailored to your family, your assets, and your goals — rather than a generic template.
You complete the questionnaire in the comfort of your own home, at your own pace. There's no time pressure — take as long as you need to think through your answers and discuss them with your partner if applicable.
Once we receive your completed questionnaire, we use it to prepare your documents. We'll review everything with you before you sign, so there's always an opportunity to make changes.
What Information You'll Need
Don't worry if you don't have everything right away — complete what you can and come back to the rest later.
Full legal names (as on your passport or birth certificate), dates of birth, home address, email, and phone number. If you're completing the questionnaire as a couple, you'll need these details for both of you.
Full names and dates of birth of your children and grandchildren. Details of any previous relationships, step-children, or dependants. Names and contact details of anyone you'd like to appoint as guardian for minor children.
The people you'd like to appoint as executors of your will, trustees of your trust (if applicable), and attorneys under your enduring powers of attorney. For each person, we need their full legal name, date of birth, address, phone, email, and relationship to you. You should also choose backup appointees in case your first choice is unable to act.
A list of your significant assets — property, bank accounts, investments, KiwiSaver, business interests, vehicles, and valuable personal items. Details of any mortgages, loans, or other liabilities. If you have an existing trust, details of the trust and the assets it holds.
Who should inherit your assets and in what proportions. Any specific gifts or bequests. Your wishes about healthcare, funeral arrangements, and organ donation. Any special considerations — such as vulnerable beneficiaries, charitable giving, or family situations that need careful handling.
Email accounts, social media, online banking, subscriptions, cryptocurrency, and any other digital assets or accounts. This helps your executors find and manage your online life.
